In response to your request to send you the list of the requirements in taking the licensure examination from the PRC, I quoted part of the Exam-Application page of the PRC’s official website:

   1. Original and photo/xerox copies of Transcript of Records with Special Order and Date of Graduation with SCANNED PICTURES AND WITH REMARKS ?FOR BOARD EXAMINATION PURPOSES ONLY?. View Res.2004-200. .(Graduates of government schools and institutions/programs accredited by recognized accrediting agencies under the FAAP are exempted from SO [B]) . Graduates of New Schools/Degree/Programs must submit School Recognition and/or Permit to Operate.
2. Original and photo/xerox copy of NSO-issued of Birth Certificate (in NSO security paper; if NSO copy not very clear, bring copy from local civil registrar)
3. Four (4) passport size colored pictures with complete name tag in white background
4. Current Community Tax Certificate (Cedula)
5. Original and photo/xerox copies of NSO-issued Marriage Contract in NSO security paper (for married female only;if NSO copy not very clear, bring copy from local civil registrar)
6. Other specific requirements as required by the Commission and/or the Professional Regulatory Board.

    Sir regarding sa Birth certificate, kailangan ba bagong kuha sa NSO? May kasama kc kami na nadeny birth certificate even if NSO security paper cya, expired na daw dahil matagal na..Gaano ba katagal before ma expire yung birth certificate?

    i just want to know what are the requirements for retaking the NLE this november 2009. deadline is on september 16 but i dont know what are the requirements needed for retakers. can’t access PRC’s official website because it is blocked in my PC because it was a threat something like that. somebody care to post here thank you.

    Ask ko lang po if ever late registration for nusing Cebu board passer ng nov2009(on March18, 2010), may kelangan bang additional papers or protocols? or will it follows the same needed docs as per normal application dates set by PRC.

    Hi! I just wanna ask something to u guys especially to those married women like me. Should i still secure my marriage affidavit (if i wasn’t mistaken) for the local board? I already am using my husband’s name in school and on my TOR as well. Or i think marriage affidavit is just for women whose surname is when they were still single? Pls reply. thnx!

    I recently pass the nursing board exam last november 2009.. i wasn’t able to register and attend the oath taking because nasa abroad po ako with my mom..

    pwede ko po bang malaman kung kailan po ang sunod na scherdule nag nursing board exam?gradute po ako nag year 2003 never ko pa pong nag try na mag board kase po nag abroad agad po ako.eh ngyun po dito ako sa pinas gusto ko pong ag try na mag board exam sa nursing.Ano po kaya mga dapat gawin kase sobrang tagal na po d ako naka pag exam.at problema ko po nawala na po yung RLE ko na notebook dun po na kalagay yung mga minor & mayor case ko delivery sa OR at DR at assist ko po.ano po kayang mapapayo nio?sana po ma sagot nio po ang mga katanungan ko bago po dumating yung susunod na schedule
