How to Apply for the SSS ID?

Question:

I became member of the Social Security System on the year 1999. I don’t have SSS ID yet. I did not bother to get the SSS ID because I thought it’s not really that important as long as my SSS monthly contribution is paid.

When I opened a bank account, I was asked valid ID. That’s the time that I realized that SSS ID is important.

I want to apply for the SSS ID and to get it. Can you please tell me what are the steps?

Answer:

Here’s what you need to do.

  1. Get E-6 Form. You can get it at the guard of the SSS office near you or just download it, print it and fill it up with prope information.
    Note: If’ you’re asked the employer number, get the number using the  SSS Online Inquiry system. Read the articles, “Philippine SSS Website Registration” and “How to get the SSS Online Inquiry Username and password?” for more information.
  2. Visit the SSS Office. Of course, you can go to the nearest SSS office. Then tell the guard that you want to apply for the SSS ID and that you already have filled the E-6 form. And follow what the guard will tell you.

You’ll learn the next steps at the SSS Office. Just make sure to talk with the guard-on-duty. Just be patience, calm, and talk the SSS personnel politely.

    Gusto ko sanang irenew itng SSS ID ko dahil yong luma pa. Nagpunta ako sa opisina ng SSS sa Urdaneta City, Pangasinan, sabi nila sa Maynila na raw. Ang problema ko paano ngayon , bakit wala na bang ibang paraan. Kaiulangan kong tulong ninyo.

      dapat po kumuha kau ng affidafit of lost kung binan branch lang po punta kau ke vise. free naman ang notary dun. hinde kc pwede maraming number id sa sss. like my brother case. mdming papers need to claime the original account sayang contribution mo pg gnun po. try my advice, nwala din atm card ko last undoy, ok lang i have xerox copy to claim one. but for sure update mo agad. habol ko lang naman sa sss if im going to die i have free burial. nothing less. sure my gnun free tlag. sa goverment my additional help pa if mhirap ka, kung myaman k naman, dont mind n cla. madaming documents to claim your right,, hehehe. good luck po.

      I suggest you call SSS office. Upon your call, prepare your stub, may mga info na they will ask na dun mo makikita…Para malaman mo where to get your ID or why di mo pa ito nakukuha..Yung skin nga 2004 ko apply, di dumating i found out sa website nila that they addressed it in Manila. Not in my province..Wrong info ng encoder ang na-input.! Now, di pa rin ako apply for a new one. Kasi i will be needing Affidavit of Loss (notary fee 150),
      replacement fee 150 rin..magastos! Dhil sa error nila ako ang suffer!
