Hello,

I was married last May 2012. Pwede po mag-ask, kung ano ang requirements in changing my marital status in my driver license. And should I change my name too?

If you want to report to the LTO the change of your marital status, just do these:

  1. Photocopy your marriage certificate.
  2. Visit the nearest LTO office bringing your Driver License Card and photocopy of your marriage certificate.
  3. Secure a Driver License Application Form from the guard or from the support or info corner, and fill the form. Make sure that you properly indicate your marital status on that form.
  4. Then submit the form along with your marriage certificate and Driver License card to the proper LTO-clerk for the processing.

Regarding to your question whether to change your name or not… you actually have the option not to change your name.

If you don’t want to change your name take note of these:

  1. In the Driver License Application Form, fill the Name field with your name, the Middle Name with the family name of your mother, and the Family Name Field with the family name of your father.
  2. If the form asked you your maiden name, just leave it blank. Why? It is because you don’t have a maiden name since you did not change your name to carry the family name of your husband. You will only have maiden name if you will change your name and in this case, the family name of your husband will become your family name and your original family name will become your maiden name.
  3. And make sure that you check the “Married” in the Marital Status option.
